Recording Your TV Viewing
Step 4: Please write down these three pieces of information any time the TV is watched for 5 minutes or more.
Use this column in your diary to mark an "X" and draw a line down to indicate when the TV set is turned on.
Write in the station or network call letters which usually consist of three or four letters. Please write in the channel number you enter on your TV to select the station.
Enter the name of the program being watched. If you do not know the name of the program, please refer to a newspaper program listing or TV guide. If you cannot find the name, write in any information that will help us identify what you were watching.
Let us know who watched or listened to the program by writing an X and drawing a line down to show who watched/listened and for how long.
